Membership fee;
There is a membership application fee of $10.00 to be submitted with the application. Application fee is to be credited
as follows: $8.00 to General Fund for payment of Death Benefits. $2.00 for Secretary - Treasurer's Fund for necessary expenses
incurred in the performance of duties.
Assessment of members;
Upon death of four (4) members,in good standing with theis association, each of the surviving members of this association
shall be assessed the sum of five (5) dollars.
Payment of Benefits;
Upon the death of a memeber in good standing in this association, the beneficiary of the member shall be paid a death
benefit computed at the rate of one dollar for each surviving member in good standing in this association.
Proper notice must be given to the Secretary- Treasurer or to a member of the Board of Trustees upon the death of a member.
Beneficiaries;
Each applicant shall, at the time of applying for membership, submit the names and address of the beneficiaries and contingent
beneficiary. You as a member may change beneficiaries at any time with written notice to the Secretary - Treasurer.
You also receive a membership card and a complete copy of the association by-laws.
